Self-Care Checklist
This checklist will help you to determine your health condition. Please read each question carefully, and then write T (true) or F (false) on the line next to the question.
1._________________ You have a busy working schedule and feel stressful sometime.
2. _________________ There is some headache or migraine attack.
3. _________________ You feel some pain or ache come and go.
4. _________________ When you forget to stretch you could feel stiffness in your joints.
5. _________________ There is some problem to get into sleep or wake up at mid night.
6. _________________ Feel low energy and reluctant to exercise.
7. _________________ No appetite for food or suffer too much food craving.
8. _________________ Sweat a lot, some times have night sweat.
9. _________________ Have blood pressure problems over 3 months.
10. _________________ Feel cold all the time, especially for hands and feet.
11._________________ Your bowel movement is not regular.
12._________________ Vision or hearing is not as good, sometimes suffer dizziness or palpitation.
13. _________________ I think I am overweight.
14. _________________ Short term memory seems getting worse.
15. _________________ I do drink quite a lot water everyday, but still feel thirsty.
16. _________________ Had surgery before, or some family history of certain disease.
17. _________________ If female, have irregular menstrual cycle.
18. _________________ Never try any detoxing program.
Now look at your answers, if you have over 5 T(true) you’d better consider a treatment for your problem; if you have over 3 T(true) a clinic consultation will help you.
Pass the self-care check with only 1 or 2 T(true), you might take good care of yourself, keep going and try the checklist 90 days later.
